Under-18 Treatment Policy

  • 1 A parent or legal guardian must complete and sign the consent form before the appointment can proceed.
  • 2 The signed form must be presented on the day of the appointment — either printed or as a clear photo/scan.
  • 3 A patch test is recommended at least 24–48 hours prior to the appointment for all first-time clients.
